As states gain experience in value-based payment, they may consider implementing prospective payment models that support practices in offering more comprehensive care, such as through integrating physical and behavioral health care, addressing health-related social needs, and creating ADVANCING INTEGRATED MODELS Advancing Integrated Models (AIM), led by the Center for Health Care Strategies and made possible through support from the Robert Wood Johnson Foundation, is a national multi-site demonstration assisting health systems and community providers in integrating innovative, “next-generation” person-centered approaches for individuals with INTEGRATED CARE PROGRAM: PERFORMANCE MEASURES RECOMMENDATIONS Center for Health Care Strategies Integrated Care Program: Performance Measures Recommendations – 3 Introduction oday, more than seven million Americans are defined as "dual eligibles" – low-income people who are elderly or have disabilities and are covered by both Medicare and Medicaid. IT’S NOT JUST RISK: WHY THE SHIFT TO VALUE-BASED PAYMENT The movement to adopt value-based payment (VBP) in the U.S. health care system tends to focus on getting providers to assume financial risk.
