YOUR HOSPITAL'S SAFETY GRADE The Leapfrog Hospital Safety Grade is a public service provided by The Leapfrog Group, an independent nonprofit organization committed to driving quality, safety, and transparency in the U.S. health system. SPRING 2021 STRAIGHT "A" HOSPITALS SINCE 2012 Spring 2021 Straight “A” Hospitals Since 2012. Leapfrog is proud to recognize the 27 hospitals across the U.S. have achieved 19 consecutive “A” grades, which represents an “A” in every biannual grading cycle since the launch of the Safety Grade in spring2012.
SCORING METHODOLOGY
The measure set is divided into two domains: (1) Process/Structural Measures and (2) Outcome Measures. Each domain represents 50% oftheSafety Grade .
